After locating you, we will send you a recovery agreement and limited power of attorney form. Once you have received the forms, simply sign them, and return them to us by fax, email or US Mail. When we receive your signed forms we will immediately begin work on your claim, and will keep you notified of its status. In the event that additional information is needed, one of our investigators will assist you with the documentation necessary to complete the claim.
